Case Studies, Chapter 4, Common Reproductive Issues
1. Terri, age 33, has been married for 2 years. She and her husband Richie have been trying to get pregnant since they first got married. Richie could not be at this appointment due to work commitment. Terri seems depressed as she discusses her infertility problems and starts to cry, wondering if she will ever have a baby. Her periods started when she was 13 years old. They have always been irregular, usually every 18 to 22 days, and last as long as 10 days. Most of the time Terri experiences extremely heavy menstrual bleeding. When she was 14, her appendix ruptured. At age 19, she had part of her left ovary removed due to a cyst measuring 12 cm in diameter. (Learning Objectives 1, 2, and 3)
a. Discuss probable causes for Terri's infertility.
b. Discuss nursing interventions related to Terri's reproductive
disorders.
2. Karen is age 40 and has been married for 22 years. Karen started having menstrual cycles at age 12 and had her first child at age 16. She has four children
ages 16 to 24. Karen's menstrual periods have always been "heavy" but regular between 28 and 31 days apart. Over the last 6 months she has been experiencing severe menstrual bleeding with progressively worsening back pain and cramping. Karen was seen by her family doctor and a CBC revealed a hemoglobin of 8.9. She was referred to an OB/GYN who after extensive diagnostic testing and a physical examination determined that she should recommend a complete hysterectomy. (Learning Objectives 1, 2, 3, and 6)
a. What common reproductive concern is the probable cause of Karen's
symptoms including severe menorrhagia, progressively worsening dysmenorrhea, and anemia?
b. What diagnostic tests do you think the OB/GYN ordered before concluding
that a complete hysterectomy was in order?
c. Discuss the nursing management that Karen will need in order to assist her
in understanding the physiologic and psychological conditions she will experience after her surgical menopause.
